Before your first visit to us, we ask you to fill out our medical history form (see https://neurologie-afradi.de/unser-anamnesebogen/ or alternatively under "Downloads"), sign it and send it to us (by email, fax or hand delivery) at least 48 hours (2 working days, Mon-Fri) before your appointment. The medical history form also serves as a treatment contract that applies to your possible subsequent visits. Please understand that your agreed appointment will be cancelled by us if the medical history form is not received by us 48 hours (2 working days, Mon-Fri) before the agreed appointment. Please understand that we may cancel your appointment if we do not receive the medical history form 48 hours (2 working days, Mon-Fri) before the agreed appointment. As an appointment clinic, we can better organize our daily routine and be there for people who need a timely visit to a specialist this way.
We kindly ask you to cancel your appointment in good time (maximum 48 hours before the appointment) if you cannot/do not want to keep it. Canceling your appointment in good time gives us the opportunity to better organize our practice processes and avoid a potential gap in our appointment calendar: this allows us to help other people who sometimes have to wait weeks for an appointment. Please note that a cancellation fee will be charged if such an appointment is not canceled in good time.
Please bring any documents (doctor's letters, findings, CDs, medication plan) that may be relevant to your current symptoms to your appointment. We then decide on site which documents we actually need.

Since we treat a large number of patients with chronic and serious illnesses and unforeseen emergency complaints can therefore always occur, we cannot always avoid waiting times in the practice, despite precise scheduling. We ask for your understanding.

Please note that consultation times differ from opening times. Our door remains closed outside of our opening times. If you would like to visit our practice in person outside of these times, please contact us in advance (by phone, email or in person).

Due to a lack of staff, we are unfortunately unable to be available to you by phone around the clock. If you cannot reach us by phone, you can send us your request by email. We will process the emails as quickly as possible and respond to them if necessary.
Note on our open office hours:
The "Open Consultation Hours" give people the opportunity to bypass the waiting time until a planned appointment if they suffer from more acute symptoms. Please note that this is only about the complaints that should be treated acutely from a medical point of view. We ask that you refrain from coming to the open consultation hours if your symptoms persist or do not represent a life-threatening situation. It should be pointed out that the open consultation hours are “crisis management/emergency treatment”. We imply that we have a limited daily capacity to treat patients in the open office hours.
You need an appointment, but the earliest possible appointment is too late for you?
Aufgrund der hohen hausärztliche Nachfrage und um die schnellstmögliche Abklärung/Behandlung der Patienten mit akuteren Beschwerden (sog. „Akutfall“) gewährzuleisten, haben wir feste Termine für die Akutfälle vorgesehen. Dies führt dazu, dass wir nicht all der verfügbaren Termine zur Online-Buchung freigeben können, was spätere Terminmöglichkeiten zur Folge haben kann.
Sie haben folgende Möglichkeiten:
- You ask your family physician to arrange an urgent appointment for you with us (a so-called family doctor referral case) if there is a need for a quick appointment from the family physician's point of view.
- You ask your family physician/referring doctor to stick an "emergency code" on the referral letter, if there is an indication for this. You can then use the emergency code to arrange a quick appointment via the Associan of Health Insurance Doctors (hotline number: 116117).
- You can put yourself on the waiting list on the Doctolib app and move your appointment up if someone with an earlier appointment cancels.
- If you have exhausted all of the above options and are still not making any progress, you can contact us by email and describe your concerns; we will put you on our waiting list and give you priority if an appointment becomes available at short notice.
